Care & Connect provides cognitive and tactile activities to adults with moderate to severe dementia in a residential setting. Our activity sessions are conducted as a group but are individualized to each participants abilities. Topics and all worksheets are tailored to an adult population which focus on oral, written, listening, small and large muscle skills.
Care & Connect provides family outreach materials that documents resident activities, identifies their current abilities and appropriate activities for engagement. Photographs of residents are included.
Care & Connect conducts inservice training for professional caregivers in the Care & Connect model and supportive engagement with residents.
